Family, friends, physicians or other healthcare agencies can make referrals. Admission to hospice is based on need and the recommendation of the patient’s physician. The following information is needed from the patient’s primary physician: Doctor’s orders, demographic information, insurance information, History and Physical information, and the most recent progress notes.
