Clients with a chronic long-term illness of all racial/ethnic backgrounds, genders, and economic status may qualify for this service. The Respite Program Manager makes an assessment visit to the caregiver and client, completes appropriate forms, then a “match” visit is scheduled with the caregiver, client, volunteer, and the Respite Program Manager. A service agreement is signed and all involved and a mutually agreed upon schedule for weekly visits is set up. After the volunteer has been matched with a client, a 30-day follow-up form is completed by the caregiver and the volunteer. A reassessment visit occurs every six months.
