Social Work Support

One of the first contacts the patient or family has through Mercy Hospice is one of our seasoned and experienced Social Workers. The hospice social worker is an expert in community resources and is able to measure patient and family needs and direct them to agencies that may be able to help with transportation, meals, financial, and legal resources. The purpose of this initial visit is to assess the psychological and social needs of the patient and family. The social worker has expertise in counseling and will provide support and guidance with issues surrounding the care of the patient and the needs of the family.

After the initial visit, an assessment is made and a Plan of Care is created. The Social Worker will seek to include the family in sharing caregiving tasks and assist them in planning for the approaching death emotionally and financially. Additional visits assess changes in the patient’s and family’s needs and are followed up with an updated Plan of Care. Our Social Worker seeks to help the patient and family with current problematic situations, unresolved issues from the past, and scenarios which may occur in the future after the death of the loved one.
